<br /> r <br /> C~DESIGN? <br /> September 19, 2005 <br /> City of Eugene, Public Works Engineering Division <br /> 244 E Broadway <br /> Eugene, OR 97401 <br /> Attention: Mr. Matt Rodrigues <br /> Proposal Addendum <br /> City of Eugene PPP 2007 <br /> Hllyard Street, East 18"' Avenue to East 24"'Avenue <br /> Eugene, Oregon <br /> GeoDesign Project: Eugene-6-03-02 <br /> INTRODUCTION <br /> GeoDesign, Inc. is pleased to submit this proposal addendum for pavement engineering services <br /> for the City of Eugene, Oregon. The purpose of the addendum is to include Hilyard Street from <br /> East 19"' Avenue to East 24"' Avenue to the City of Eugene PPP 2007 list of projects. The section <br /> of Hilyard Street is approximately 2,500 feet long and one way to the north from East 23id Avenue <br /> to East 18"' Avenue. The services to be provided include pavement rehabilitation analysis <br /> consisting of falling weight deflectometer (FWD) testing, subsurface explorations, and equivalent <br /> single-axle load (ESAu calculations. <br /> ROPE OF WORK <br /> We propose to complete shallow cores and FWD testing to evaluate the existing pavement <br /> conditions. Our specific scope of services will include the following: <br /> • Complete a distress survey of the existing pavement, including logging the extent of cracking <br /> and rutting. <br /> • Explore subsurface conditions in the road right-of--way by completing up to five cores to <br /> depths of up to 5 feet below the ground surface. <br /> • Maintain a detailed log of the explorations and obtain samples of the pavement, base, and <br /> subgrade materials encountered. <br /> • Obtain soil samples at select depths in the soil borings. <br /> <br />
