PERSONAL SERVICES CONTRACT <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> AND: Balzhiser & Hubbard Engineers <br /> Contract No.: 2006-00230 <br /> This contract is being established per the terms and conditions of Personal <br /> Services Master Contract master contract #2005-00220 dated January 24, 2005, <br /> between the City of Eugene (City) and Balzhiser & Hubbard Engineers <br /> (Contractor). <br /> City and Contractor mutually agree to the attached scope of work as .outlined <br /> herein and the contract referenced above as follows: <br /> Project Description: The project consists of obtaining a Letter of Map <br /> Revision (LOMR) from the Federal Emergency Management Agency <br /> (FEMA) to address relocation of a drainage channel within the 100- <br /> year floodplain. The channel is located north of Enid Road and Auction <br /> Way and is a tributary of the A-1 Channel. Maximum total contract <br /> amount: $9,600.00.
