<br /> The Applicant activated its force account resources to undertake debris removal operations to protect public health /safety and protect improved property.
<br /> Specifically, the Applicant utilized Force account labor (approximatly 150 employees 5,167 reg time 98 OT) hrs include parks and street departments, this
<br /> required a large number of labor hours due to labor intensive hand work was required to move hazard limbs, trees and debris in various improved areas throughout
<br /> the city of Eugene,the labor hrs also includes monitoring of the contractors the monitoring of the contractors were not broken out separately from the debris
<br /> cleanup hrs., Force account equipment hours (5,303.The acquired Contractors and the applicant purchased materials to perform the work. The applicant and
<br /> contractors collected 3607.06 CY of debris which was removed and hauled to various green waste recycle facilities (Lane Forest and Rexius locations) for the final
<br /> disposal. They are participating to the Debris Pilot Program.
<br /> This PW is for the first 30 days' work.There will be additional PWs for 30 -90 and 90 -120 days
<br /> Total Cost for Work Completed is as follows:
<br /> Contract $68,865.10 (see scope notes)
<br /> Equipment 5106,632.29 the applicant is using their own equipment rates which are consistent with FEMA rates which were reviewed by the project specialist
<br /> Labor $ 189,065.78
<br /> Total Project Cost $ 364,563.17
<br /> The applicant affirms via email (attached) that (1) all cut hazard limbs were at least T diameter at the cut; (2) all leaning hazard trees were at least 6" diameter
<br /> at 4.5' above the ground; (3) all hangers and leaners posed a threat to public health and safety if not removed. Addresses of the approximate locations where this
<br /> work was performed are given on the contract billings, which are attached. This is in accordance to their policy.
<br /> SCOPE NOTES
<br /> The applicants policy allows them to contract for goods Et services, up to 575,000, using informal solicitation process (verbal quotes). This process requires the
<br /> applicant to get three quotes, which they state they did. This process is described in article 10 of the applicants purchasing regulations, page 43, article 10. The
<br /> applicant states they already had an established contract with CTS Services. This contract included the scope of work they needed for storm response and a
<br /> pricing schedule, to get additional contracts in place, the applicants lead worker reached out to many other contractors to get verbal quotes on prices, and most
<br /> chose not to respond due their availability. They were already swamped with private work, they couldn't take on additional work from the City.
<br /> According to the agreements between the contractors and the City of Eugene the debris monitoring wilt be done by the city. The city monitors were in the field
<br /> with the contractors. The city monitors periodically checked on the contractors that were performing the work, they also checked the area after the work was
<br /> completed for validation.
<br /> The applicant utilized out of class personnel to perform tasks such as emergency calls from the public, and site visits of reported hazardous tree debris, because
<br /> of staff limitations and magnitude of reports to perform this work. The salary rate applied for FEMA reimbursement for these employees was based on the salary
<br /> rate of an employee normally performing these tasks and not the higher salary rate of the temporarily assigned employee.
<br /> Applicant shall obtain and document compliance for any required coordination, permits, and /or consultation with applicable regulatory agencies. This
<br /> documentation may be required for closeout. Failure to do so may jeopardize Public Assistance funds.
<br /> As described in 44 CFR 13.43 and to accordance with State requirements, applicant must maintain all project related records for a period of six (6) years from
<br /> applicant closure.
<br /> No mitigation opportunities have been identified. This project worksheet is for emergency work only; therefore mitigation is not eligible.
<br /> The applicant is not requesting Direct Administrative Costs that are directly chargeable to this specific project.
<br /> Any change to the approved scope of work on a Project Worksheet (PW) must be reported to Oregon Office of Emergency Management and approved before work
<br /> begins. Failure to report changes may jeopardize federal funding.
<br /> The applicant is required to adhere to local, state, and at a minimum, federal procurement requirements for all purchases of goods and materials and contracting
<br /> services funded under this project per 44 CFR 13.36.
<br /> The applicant is responsible for obtaining all applicable Federal, State and local permits and compliance with applicable laws for removal, staging, processing,
<br /> hauling, disposal or burning of debris for this disaster at an approved landfill or disposal site
<br /> Accelerated Debris Removal (85% Federal Share): Sub- grantee has chosen to participate in the Accelerated Debris Removal Alternative Procedure. All work was
<br /> completed within the first 30 days after the start of the Incident period, reimbursable at the authorized 85% Federal Share.
<br /> Force Account Labor (Straight - Time): Sub grantee has chosen to participate in the Straight -Time Force Account Labor Alternative Procedure for Debris Removal.
<br /> Scope of Work: As a result, straight -time force account labor, including fringe benefits, will be reimbursed on the PW.
