DATE CATEGORY FEMA 14169 I I DR 1-OR JC00009 039 - 23850 -00 05 -17 -2014 A APPLICANT: EUGENE WORK COMPLETE AS OF: 05 -17 -2014: 100 % Site 1 of 1 DAMAGED FACILITY: COUNTY: Lane Debris Removal LOCATION: LATITUDE: LONGITUDE: 44.0618 - 123.11919 PA-10-0R-4169-PW-00006(0): City Hall 1820 Roervalt Ave, Eugene OR DAMAGE DESCRIPTION AND DIMENSIONS: PA-10-0R-4169-PW-00006(0): A severe winter storm occurring February 6 -10, 2014, brought a mixture of arctic air, strong east winds, and significant snowfall along with freezing rain and ice which caused damage to City of Eugene. These conditions caused accumulation of 3607.06 CY of vegetative debris upon the Applicant's public properties and various 700 miles of public right of way, trails,roads and other improved areas that hazard to the public. SCOPE OF WORK: PA-10-0R-4169-PW-00006(0): WORK COMPLETED The Applicant activated its force account resources to undertake debris removal operations to protect public health /safety and protect improved property. Specifically, the Applicant utilized Force account labor (approximatly 150 employees 5,167 reg time 98 OT) hrs include parks and street departments, this required a large number of labor hours due to labor intensive hand work was required to move hazard limbs, trees and debris in various improved areas throughout the city of Eugene,the labor hrs also includes monitoring of the contractors the monitoring of the contractors were not broken out separately from the debris cleanup hrs., Force account equipment hours (5,303.The acquired Contractors and the applicant purchased materials to perform the work. The applicant and contractors collected 3607.06 CY of debris which was removed and hauled to various green waste recycle facilities (Lane Forest and Rexius locations) for the final disposal. They are participating in the Debris Pilot Program. This PW is for the first 30 days' work.There will be additional PWs for 30 -90 and 90 -120 days Total Cost for Work Completed is as follows: Contract $68,865.10 (see scope notes) Equipment $106,632.29 the applicant is using their own equipment rates which are consistent with FEMA rates which were reviewed by the project specialist Labor $ 189,065.78 Total Project Cost $ 364,563.17 The applicant affirms via email (attached) that (1) all cut hazard limbs were at least 2" diameter at the cut; (2) all leaning hazard trees were at least 6" diameter at 4.5' above the ground; (3) all hangers and leaners posed a threat to public health and safety if not removed. Addresses of the approximate locations where this work was performed are given on the contract billings, which are attached. This is in accordance to their policy. SCOPE NOTES The applicants policy allows them to contract for goods & services, up to $75,000, using informal solicitation process (verbal quotes). This process requires the applicant to get three quotes, which they state they did. This process is described in article 10 of the applicant's purchasing regulations, page 43, article 10. The applicant states they already had an established contract with CTS Services. This contract included the scope of work they needed for storm response and a pricing schedule, to get additional contracts in place, the applicants lead worker reached out to many other contractors to get verbal quotes on prices, and most chose not to respond due their availability. They were already swamped with private work, they couldn't take on additional work from the City. According to the agreements between the contractors and the City of Eugene the debris monitoring will be done by the city. The city monitors were in the field with the contractors. The city monitors periodically checked on the contractors that were performing the work, they also checked the area after the work was completed for validation.
