ICS FORMS <br /> ICS 206 Medical Plan <br /> The Medical Plan is intended to serve incident personnel. On <br /> incidents where medical aid to the public is required, a Medical <br /> Division, Group, or Branch will be assigned to Operations with its <br /> own resources and assignments. In such cases, the medical needs of <br /> incident personnel may be met by this organization, and no "incident" <br /> plan would be required. This form may be used by the Medical <br /> Division /Group or Branch as a working tool. <br /> ICS 214 Unit Log <br /> The Unit Log is for use by any level supervisory personnel to <br /> document the major events that occur within their respective <br /> organization during the incident. This Log assists in conducting the <br /> briefing to the supervisor for the next operational period, and in <br /> conducting the debriefings with the Documentation Unit of the <br /> Planning Section. All of the Unit Logs for an incident will assist <br /> in the Incident Debriefing following the conclusion of the incident. <br /> ICS 215 Operational Planning Work Sheet • <br /> The ICS 215 is used by the Planning Section to develop a listing of <br /> the resources required for each operational period on a long term <br /> incident. The use of this form is covered in greater depth in the <br /> Planning Section course. <br /> PAGE -2 <br />
