ICS FORMS <br /> INITIAL ATTACK INCIDENT COMMANDER <br /> ICS 201 Incident Briefing <br /> The four part ICS 201 form is intended for use by the Initial <br /> Attack Incident Commander to use when briefing an incoming Incident <br /> Commander. The form has areas to record a map sketch of the <br /> incident, a summary of the current actions, the current incident <br /> organization, and a resource summary. When the incoming Incident <br /> Commander arrives at the scene of the incident, the Initial Attack <br /> Incident Commander briefs the new IC and furnishes them a completed <br /> copy of the form. From this four part form, the new IC can develop <br /> the written Incident Action Plan. <br /> WRITTEN INCIDENT ACTION PLAN <br /> ICS 202 Incident Objectives <br /> This is the key to effective action. It is the initiator of the <br /> planning and control process, and the place where Command begins to <br /> form the Command and General Staff needed, and to direct the <br /> organization. The form should describe all desired final outcomes; <br /> it should not dictate tasks or tactical actions, which are outlined <br /> later. Although the form is called "objectives ", its real purpose <br /> is to describe the goals of the organization. <br /> ICS 203 Organization Assignment List <br /> Records the key members of the organization. This form offers a <br /> quick reference to names and agencies of the key players. <br /> ICS 204 Division /Group Assignment <br /> The ICS 204 provides more detailed instructions to the "doers ". This <br /> is the place where actual tasks are described, and may be used to <br /> further define priorities. The preparation of these forms also <br /> forces another examination of the capability to meet the objectives. <br /> The forms specify resources assigned to the Division or Group, their <br /> configuration, and who does what. The actual organization begins to <br /> be visible at this point, and span of control considerations can be <br /> reviewed. Communications information is also included. <br /> ICS 205 Communications Plan <br /> This form should be completed for complex, multi- agency incidents. <br /> Its preparation and use will materially improve incident <br /> communications regardless of the types of capabilities of existing <br /> radio systems. Complete preparation of the plan will often show how <br /> additional resources (free- standing radio systems, cellular phones, <br /> etc.) can assist in meeting incident communications needs. <br /> Simple incidents may use the communications information at the bottom <br /> of the ICS 204 as a substitute for the ICS 205. <br /> PAGE -1 <br />
