TREE INCIDENT FORM <br /> DATE OF INCIDENT: 7/26/94 ADDRESS: 3rd. & Jackson <br /> INCIDENT DESCRIPTION: Large limb, 10 to 12 ", torn from tree and hanging. <br /> Reported by EPD at 1:55 pm. According to a witness, the limb was hit by a <br /> truck and broke away from tree. The attachment was sound and had no evidence <br /> of decay. Mark Watson and Dave Edwards responded and cleaned up the limb. <br /> EARLY DETECTION: This limb had no signs of failure at it's attachment to the <br /> trunk and would not have come down if it hadn't been hit.
