I mailed <br /> you the form after our meeting but I haven't received it back yet. As I stated to you, any permission to <br /> remove a street tree is based on the City receiving the request for removal on the permit form first and <br /> then, if approved, the value of the tree is given to the City along with other conditions if appropriate. <br /> Please complete the tree removal permit form and send or deliver it to my office at PWM 1820 Roosevelt <br /> Blvd. Once I receive it, I can process it.
