Box 1949 <br /> Eugene, Or 97440 <br /> NOTICE OF CIVIL PENALTY <br /> Notice is hereby given that on December 11, 1993, the City of Eugene is imposing <br /> - against you a civil penalty in the amount of $1000.00. <br /> This penalty is being imposed due to violation of Section 6.305(2) of the Eugene Code <br /> occurring on property commonly referred to as tax lot 18042114 - 01000, 3620 Whitten <br /> Dr., Eugene, Oregon. The violation can briefly be described as removal of at least 10 <br /> trees from the property without a permit. <br /> This Notice of Civil Penalty is issued without prior written notice to correct the violation <br /> on the basis that the violation is complete and not correctable. <br /> You may request a hearing by filing a notice of appeal at the Permit and Information <br /> Center, 244 East Broadway, Eugene, OR 97401, within 15 days of the date of this <br /> letter. The notice of appeal must be completed, signed and accompanied by an <br /> appeal fee of $100.00 The appeal is not effective until all of these requirements have <br /> been completed. <br /> You may appeal the imposition of these civil penalties, including the determination that <br /> a violation exists, that you are a responsible party, or the amount of the penalty. <br /> Unless the City receives your request for a hearing within 15 days of the date of this <br /> letter, you waive your right to a hearing. <br /> A civil penalty becomes final upon the expiration of the time for filing an appeal, if an <br /> appeal is not filed.
