SAFETY AWARDSANCENTIVES <br />1 Year Awards - Awarded to all employees who work a calendar year without a <br />lost time accident. lost time is when an employee misses <br />two consecutive full work shifts. as a result of an on <br />the -job injury. Each employee receives a safety pin. <br />Progressive Award - Awarded in 5 year increments of 5, 10, 15, 20, 25, and <br />30 years to any worker who has worked 5 years or more con- <br />secutively without a lost time accident. Each employee <br />has their name engraved on a metal plate and mounted on the <br />corresponding plaque. A framed certificate is given to <br />each employee with a copy of the certificate being placed <br />in their personnel file. Finally, a safety hat is given to <br />each employee receiving a progressive award. <br />Award of Excellence - This is recognition given by the Public Works Director to <br />an individual or section. A crew award is given to a crew <br />that collectively works one calendar year without a lost <br />time accident.. <br />Safety Incentives Eligibility Requirements - <br />Door Prize - Anyone eligible for recognition in the safety awards <br />program. (2 prizes) <br />1 Year Prize - , receiving a l year award is eligible. (2 prizes) <br />Progressive Prize Anyone who has ever received a progressive award, 1 <br />chance for every 5 years earned. (2 prizes) <br />Winners of above prizes are drawn and only 1 prize per employee. Employees must <br />be present to win a prize. <br />
