Medlin, <br />Parks and Open Space Division Manager <br />Jackie Hallett, <br />Volunteer Program Supervisor <br />2010 Neighborhood Matching Grant: Safety Education <br />Plaza at Bailey Hill Road in Churchill Sports Park <br />Attached for your review and signature, are two original Statement of Terms and Condition <br />Agreements for Safety Education Plaza at Bailey Hill Road in Churchill Sports Park. This <br />project was approved and funded through the City's Neighborhood Services Program and <br />will be implemented by POS Volunteer Program staff. <br />The scope of the project. is to build a safety education plaza near; the new `Z' crosswalk on <br />Bailey Hill Road. The plaza will have a bronze memorial plaque honoring Vaclav Hajek, <br />two benches and a two sided safety information sign. The project purpose is to create <br />healing and community through on-going safety education. <br />Exhibit B provides a revised budget as approved by City Council. The City's grant match <br />portion is not to exceed $4,561.00. <br />
