I have printed up the forms for you – but I’m confirming the rates with Roberta Pupilli first (I’ll let you know when I’ve got that confirmation). The current rates that I can find are either $11.54 or $18.46. I’m not sure if those are correct for the iphone though. Before I get too far into this though – I just need to make sure your service provider is Verizon. Apparently, we can only do the stipend when your carrier is Verizon. Can you confirm that your carrier is Verizon? Also – we will need in writing something that documents the need for the stipend from your supervisor, and then I will forward all of that on to Mark for approval. Once we get his approval – I’ll submit the paperwork to Stephanie so you can be PAF’d.
