COMMERCIAL DRIVER'S LICENSE <br />The following are the agreed upon practices and procedures, which will be followed when an <br />employee's job duties require them to posses a Commercial Driver's License '(CDL). These <br />procedures apply solely to the. acquisition of a Commercial Driver's License and not to any other type <br />of driver's license. <br />1) The City will provide the equipment for the employee to take the driving test as well as a <br />qualified driver to drive the employee to the testing site.. The time taken by the employee to <br />do this is considered time worked. In the event the employee's regular work shift is not <br />during the testing hours, the supervisor may arrange for flex time or overtime depending on . <br />the needs of the work group. <br />2) The employee must pay the. licensing fees, renewal fees, and any other associated fees for <br />tests. The renewal license must be obtained by the employee on his/her own time as with <br />any other type of driver's license. <br />3) The City will pay the cost of the required physical examination if the employee sees a <br />physician at the occupational medical clinic the City has contracted with, Cascade <br />Occupational Health. Supervisors, should make appointments for these exams directly with <br />Cascade Occupational Health,541 -228-3 100. <br />4) If an employee chooses to have the physical examination by a physician other than one at . <br />Cascade Occupational Health, the City will reimburse the employee for an amount equal to <br />the fee paid to Cascade Occupational Health for an ICC exam. To receive reimbursement; <br />employees will need to submit a copy of their statement; clearly showing the charge for an <br />DOT physical, to Risk Services, along with a copy of the Medical Examiner's Certificate. <br />NOTE: This type of exam is specifically excluded from coverage by health plans. <br />5) Physical examinations .required for CDL holders are valid for two years, under most <br />circumstances, and must be. kept current Re- examination procedures and policies are <br />identical to, those governing the initial physical examination. This includes the practice of <br />reimbursing employees if they choose to be examined by a physician other than at Cascade <br />Occupational Health. When the re- examination physical is done by a physician other than at <br />Cascade Occupational Health, a copy of the Medical Examiner's Certificate should be sent to <br />Risk Services. <br />
