After Ryan fills out his information, return the form to me, as I will need to forward to Roberta for her portion <br />before forwarding to Verizon. Additionally, it is my understanding that Ryan would turn in his COE Verizon phone, and the <br />phone number would be transferred to a new Verizon phone which he will purchase. (Clara -- didn't we have this situation once before ?) I believe there is a form <br />that you can submit to transfer liability for a specific city number to a personal number while retaining that same number for <br />your use. Of course, you can always opt for the new number if that is your preference. It has contact information and <br />directions on how to get the discount. If you do opt to go with your own cell phone number, you will need your supervisor's <br />approval for the stipend. I would speak with them before proceeding.
