<br />EXHIBIT E-7 CERTIFICATE A <br /> <br />CERTIFICATION OF NONSEGREGATED FACILITIES <br /> <br /> <br /> <br />Applicable to government contracts and subcontracts exceeding $10,000 and not exempt from the <br />provisions of the Equal Opportunity Clause as provided by Executive Order No. 11246 and <br />regulations promulgated thereunder. <br /> <br /> <br />The undersigned hereby certifies that he does not and will not maintain any facilities for his <br />employees in a segregated manner or permit his employees to perform their services at any <br />location under his control where segregated facilities are maintained. The undersigned <br />understands that segregated facilities means those which are in fact segregated on the basis of <br />A@ <br />race, religion, color, or national origin, or because of habit, local custom, or otherwise. The <br />undersigned further understands that facilities includes waiting rooms, work areas, restaurants <br />A@ <br />and other eating areas, time clocks, rest rooms, washrooms, locker rooms and other storage or <br />dressing areas, parking lots, drinking fountains, recreation or entertaining areas, transportation, <br />and housing facilities provided for employees. <br /> <br />The undersigned understands and agrees that a breach of this assurance is a violation of the <br />Equal Opportunity Clause required by Executive Order No. 11246 of September 24, 1965. <br /> <br />The undersigned further obligates him/herself to obtain and retain a substantially similar assurance <br />for each subcontract or for specific time periods (i.e., quarterly, semi-annually, or annually) from <br />proposed subcontractors prior to the award for any subcontract exceeding $10,000 and not <br />otherwise exempt from the requirements of the Equal Opportunity Clause as provided by Executive <br />Order No. 11246 and regulations promulgated there under. <br /> <br />The undersigned further acknowledges an obligation of contractors, subcontractors, and suppliers <br />having 50 or more employees and an agreement, purchase order, or contract in excess of $50,000 <br />to report annually on Standard Form 100 (EEO-1) and to develop and maintain for each of its <br />establishments a written Affirmative Action Program meeting the requirements prescribed by the <br />regulations published in Title 41, Chapter 60 of the Code of Federal Regulations. <br /> <br />This certification shall be valid for a period of one year from the date of signing. <br /> <br />NOTE: The penalty for making false statements is prescribed in 18 U.S.C.1001. <br />& <br /> <br />Prime & Sub-Contractor to sign <br /> <br />Contractor Name: ____________________ <br /> <br /> <br />Subcontractor Name: ___________________ <br /> <br /> <br /> Date ____________Signature _______________________Title:_______________________ <br />Signature contractor or subcontractor as appropriate <br /> <br /> <br />
