It is a notification regarding one <br />Eugene industry in Significant Noncompliance (SNC) of their wastewater discharge permit during calendar year <br />2010. The publication of industries in Significant Noncompliance is an every January event. Kim Mast worked on <br />this project last year. <br />The ad should appear on one weekday, on one page only in the City/Region section, either page 2, 3 or 4 <br />whichever is cheapest, on or before Monday, January 31, 2011. The ad is a similar format to that used last <br />January when we published two Eugene industries in SNC during 2009 (see attached pdf, "2009 SNC <br />3Cx5.25 . <br />PW-Wastewater needs an affidavit of publication from the Register-Guard to include in the Industrial Pretreatment <br />Program annual report to DEQ I need to receive the affidavit by Tuesday, February 15, 2011 For your reference, a <br />pdf of last year's affidavit is attached <br />The affidavit should be sent to: <br />City of Eugene PW-Wastewater <br />Attn: Sheridan Caulley <br />410 River Avenue <br />
