PERSONAL SERVICES CONTRACT <br />Architect and Engineer <br />BETWEEN: The City of Eugene, an Oregon <br />Municipal Corporation <br />AND: PBS Engineering + Environmental <br />Contract No.: 2011-00216 <br />This contract is being established per the terms and conditions of Personal <br />Services Master Contract #2009-00224 dated November 17, 2008, between the <br />City of Eugene (City) and PBS Engineering + Environmental (Contractor). <br />City and Contractor mutually agree to the attached scope of work as outlined <br />herein and the contract referenced above as follows: <br />Project description: PBS to provide an environmental assessment and <br />prepare a Hazmat Corridor Study for the Safe Routeito School project. <br />The study area includes 18 different locations in the 4J School District. <br />The work will be conducted in generally accepted environmental <br />procedures as outlined in AASHTO's Hazardous Waste Guide for Project <br />Development (1990). <br />For clarification, see Exhibit B - PBS Scope of Work Proposal <br />Contract amount not to exceed $10,340.00, with a completion date <br />of February 4, 2011. <br />All other terms and conditions of master contract #2009-00224 apply to this <br />scope of work and remain in full force and effect. <br />CITY OF EUGENE <br />/,I, bl <br />Ku Corey, P.E. <br />Pu lic Works Dir or <br />PBS Engineering + Environmental <br />, -~'a 4 - <br />Guy Nea , P.
