Please sign all three copies and return them to me at <br />the address above. When fully executed, one copy will be returned to you. <br />This contract requires you to provide the City with proof of insurance coverage in the amounts <br />listed in the contract. An insurance certificate from your insurer is adequate proof if it contains <br />the following elements. <br />-il Coverage <br />Limits <br />Expiration <br />e~k The City of Eugene listed as additional insured <br />-4, A 30-day notification period in case of cancellation <br />The contract number <br />At this time, your insurance certificate for Worker's Compensation has lapsed. The insurance <br />certificates for Comprehensive General Liability and Automobile Insurance are valid through <br />December 31, 2006. Please request a new Worker's Compensation certificate be mailed or faxed <br />to me. Then in November or early December, please request that your insurance carrier mail or <br />fax the additional certificates directly to me at the above address.
