Please attach both <br />signed copies from the contractor and start your initial process of yours and Kevin Finney's and then to Clara. <br />She will have Johnny initial. Clara, can you please give it to Sandi when Johnny has initialed it? <br />Sandi, can you send it to Karen Loy for Kurt Corey to sign, with a note to return to you. When it comes back, <br />can you forward one original with the attached green routing form to Tammy Smith to process my requistion. <br />She will forward it to CRO after. Please have your insurance carrier <br />mail to 1820 Roosevelt Blvd. Eug. 97402, or fax 541- 682 -4882 and have them reference Contract # <br />2008 - 05307. <br />The status is: <br />Automobile Liabilty Expired 7/12/07 <br />Commercial General Liability Expires 12/27/07 <br />Workers Compensation Expires 1/01/08 <br />After today, I am on vacation until the 29th.
