AMENDMENT NO. 4 <br />City of Eugene Contract No. 2005 -05301 <br />t <br />This Amendment is made to the Youth Work Crew Contract dated 07/01/2004, between the City <br />of Eugene (City) and Lane County, through the Department of Youth Service's (County). City <br />and County mutually agree to amend the contract referenced above as follows: <br />Time for Completion: The contract previously was from 7/1/2004- 6/30/2007. The contract is <br />extended for an additional year to June 30, 2008. Total contract period is 7/1/2004- 6/30/2008. <br />Consideration: Total consideration previously available was $159,000. Consideration is <br />increased by $58,000 for total consideration of $217,000. <br />- Charge at $7.80 /hour for youth <br />($7.80 /hr * 8 hours * 3 youth = $187.20) <br />($7.80 /hr * 8 hours * 4 youth = $249.60) <br />($7.80 /hr * 8 hours * 5 youth = $312.00) <br />($7.80 /hr * 8 hours * 6 youth = $374.40) <br />($7.50 /hr * 8 hours * 7 youth = $436.80) <br />($7.50 /hr * 8 hours * 8 youth = $499.20) <br />-Plus the Charge at $30 /hour for supervisor <br />($30 /hr * 8 hours = $240) <br />- Contract provides for an average daily rate of $552 ($240 for supervisor + $312 <br />for 5 youth) <br />-Daily rate covers all overhead costs including: equipment, maintenance and <br />repairs, van, fuel, insurance costs, benefits, food, telephone, administrative costs <br />and other indirect miscellaneous fees. <br />Crew Size, Work Hours, Scheduling: <br />. -Day of work consists of 8 -hour days. <br />-Crew consists of 1 qualified adult supervisor and an average of 5 youth <br />- Average yearly schedule will be 104 days of work (1 supervisor + 5;youth) <br />All other terms and conditions of this contract remain in full force and effect: <br />CITY OF EUGENE <br />Kurt Corey, P.E. <br />Public Works Director <br />Date: <br />COUNTY <br />William Van Vactor <br />County Administrator <br />Date: <br />
