REAPPROPRIATION REQUEST FORM <br />TO: Budget Office DATE: October 18, 2010 <br />Department: Public Works <br />Executive Manager Signature: <br />Please Note: General Fund Reappropriation Requests must tie directly to a specific project <br />approved by Council or the Executive Management Team in the prior fiscal year. <br />1. Description of Reappropriation Request/Justification: (attach additional pages as needed) <br />This request is to re-appropriate the funding approved in FYI 0 to perform roof repairs on the Fleet <br />Services maintenance building located at 1820 Roosevelt Blvd. <br />Due to the existing deteriorated condition of the primary Fleet Maintenance building roof surface, a <br />comprehensive maintenance and repair is required to ensure weather integrity and cessation of further <br />structural damage from water leakage. The existing Fleet maintenance building was constructed in the <br />1940's. Replacement of the Fleet maintenance building is part of the FYI 2 CIP list, but is currently an <br />unfunded prof ect. <br />Funding was approved to complete the repair in June, 2010, but a contract was not able to be completed <br />and signed with an appropriate contractor by June 30, 2010; therefore, re-appropriation of the FY10 <br />funding is requested. The contractor has begun the repairs and is currently scheduled to complete the <br />project by December, 2010. <br />2. Fiscal Information <br />3. Has a purchase order been issued on this request?
