Please fill <br />this out, sign it, and return it to us with the following: <br />1) a short legal description of the site <br />2) three full -size site plans which indicate the remaining lots to be covered by this <br />agreement (I believe those are lot #'s 6,7,11,12,13 and 22) <br />3) a $100 check to the City of Eugene for the City Plant (Option A) fee <br />4) another check to the City for a new street tree agreement totaling $307.15 ($300 plus <br />.01 /linear foot of ROW frontage -715 linear feet was my rough calculation). <br />Once I get these I will complete the plan review and forward your application to the Urban <br />Forester for approval.
