<br />Oregon 97401, for a non-refundable fee of
<br />$15,00 plus $6.50 postage. For other re-

<br />Plans and Specifications.
<br />Bids shall be accompanied by a certified
<br />check, cashier's check or blddei's bond
<br />payable to the City of Eugenia in an
<br />amount equal to ten pnr(6111. of the amount
<br />Did. Filing of a qualification questionnaire
<br />before bidding will not be required. Contin-
<br />gant or conditional bids will not be rwnsid-
<br />ered for acceptance. This public works im-
<br />provamant project is subject to payment of
<br />prevailing rate of wages, per SOLI ORS
<br />2790,800 to 2790.870. No bid will be re-
<br />ceived or considered unless the bid con-
<br />talns a statement by the bidder that provl-
<br />sionS of OR5 2790.800 to 279C.870 per-
<br />taining to prevailing wage rates shall be
<br />Complied with.
<br />Bidders are required to submit a
<br />First-Tler Subcontractor Disclosure form,
<br />Substantially as described In ORS
<br />2790.370, prior to the Bid Closing Date
<br />and Time. For this project the first-tier sub-
<br />contractor disclosure statement must be
<br />submitted regardless of the bid amount.
<br />No bid will be accepted by the City
