BID NO. 11 <br />CALL FOR BIDS <br />Notice is hereby given that the City Council of the City of Eugene, Oregon will receive sealed bids until Tuesday, <br />May 4, 2010, until the Bid Closing time of 2:00 p.m. at the office of the City Engineer, 99 East Broadway Suite <br />400, Eugene, Oregon. The Bid Opening will be at 2:05 p.m., on Tuesday, May 4, 2010, at the above location, at <br />which time and place those Bids will be opened and read. <br />Job 2010 STORMWATE' R REHABILITATION <br />4671 on 15th Avenue from Agate Street to Moss Street and at the intersection of 17th Avenue and Villard <br />Street(1LS Temporary Work Zone Traffic Control, Complete; 50 HR Flaggers;1,878 SF Concrete <br />Pavement Removal; 5,506 SF Asphalt Pavement Removal; 41 FT 10" PVC SD Pipe; 80 FT 10" <br />C900 SD Pipe; 465 FT 12" PVC SD Pipe; 289 FT 15" PVC SD Pipe; 3 EA Concrete Manhole, Flat <br />Top; 7 EA Catch Basin Trap; 1 EA Concrete Inlets, Type 4A; 5 EA Catch Basins, W/Bike Proof <br />Grate; 135 TON Level 1, 1/" Dense HMAC;1,878 SF Plain Concrete PVMT, Doweled, 7" Thick). <br />Work at the intersection of Moss Street and 15th Avenue must be completed prior to May 26, 2010. <br />Work on 15th Avenue shall be completed by June 4, 2010. All work shall, be completed June 11, 2010. <br />Plans and specifications may be reviewed and/or obtained at the office of the City Engineer, 99 East Broadway <br />Suite 400, Eugene, Oregon 97401, for a non-refundable fee of $15.00 plus $6.50 postage. Filing of a qualification questionnaire before bidding will not <br />be required. Contingent or conditional bids will not be considered for acceptance. This public works <br />improvement,project is subject to payment ofprevailing rate ofwages, per BOLI ORS 279C.800 to 279C.870. No <br />bid will be received or considered unless the bid contains a statement- by the bidder that provisions of ORS <br />279C.800 to 279C.870 pertaining to prevailing wage rates shall be complied with. <br />Bidders are required to submit a First-Tier Subcontractor Disclosure form, substantially as described in ORS <br />279C.370, prior to the Bid Closing Date and Time.
