You are now permitted to begin <br />incurring grant expenses for your 2009 - 2010 Bicyclist Safety Mini-Grant. <br />The first reporting period is contract signing (now) through to June 30, 2010. This report <br />includes request for reimbursement of expenses to date and is an opportunity to touch on <br />progress that has been made. Simply just let us know how things are going. <br />The second reporting period is July 1, 2010 - September 30, 2010. This report should <br />encapsulate the entirety of the project. This should be a more formalized report letting us know <br />how we assisted in your success, what were some challenges, reflect on outcomes, and any <br />additional thoughts you would like to share. If the project includes media, print or video, it is a <br />requirement of the grant to have me review before publication. The media needs to include a <br />reference to `Funding provided by ODOT- Transportation Safety Division.' <br />Enclosed is a reporting form called Mini-Grant Report. We will also be emailing this to you a <br />month prior to the report due date. If you wish to request reimbursement for expenses prior to the <br />reporting date deadlines, just let us know and we will send the form to you at that time. <br />Original signatures of the Authorizing Official named in the grant agreement are required to <br />process reimbursement payments. Financial documentation of expenses, ie: receipts or time <br />cards, is also required. As stated on the report forms, both reports must be submitted.
