<br />The above jobs (#4580, 4614, 4616, and
<br />4618) will be awarded as one contract.
<br />Plans and specifications may be re-
<br />viewed and/or obtained at the office of the
<br />City Engineer, 99 East Broadway, Suite
<br />400, Eugene, Oregon 97401, for a non-
<br />refundable fee of $15.00 plus $6.50 post-
<br />age. For other reviewing locations and any
<br />question regarding the project please call

<br />review Plans and Specifications.
<br />Bids shall be accompanied by a certified
<br />check, cashier's check or bidder's bond
<br />payable to the City of Eugene in an
<br />amount equal to ten percent of the amount
<br />bid. Filing of a qualification questionnaire
<br />before bidding will not be required. Contin-
<br />gent or conditional bids will not be consid-
<br />ered for acceptance. No bid will be re-
<br />ceived or considered unless the bid con-
<br />tains a statement by the bidder that provi-
<br />sions of ORS 279C.800 to 279C.870. This
<br />project is a public works improvement
<br />project and is subject to ORS279C.800 to
<br />279C.870.
<br />Bidders are required to submit a first-tier
<br />subcontractor disclosure form, substantial-
<br />ly as described in ORS 279C.370, prior to
<br />the Bid Closing Date and time. For this
<br />project the first-tier subcontractor disclo-
<br />sure statement must be submitted regard-
<br />less of the bid amount.
<br />No bid will be accepted by the City un-
<br />less It contains a statement by the bidder
<br />as a part of the bid that the bidder in carry-
<br />ing out the contract will comply with all ap-
<br />plicable laws.
<br />Work performed by the Contractor's own
<br />organization must be at least 30 percent of
<br />the awarded contract amount.

<br />The City of Eugene in accordance with
<br />Title VI of the Civil Rights Act of 1964, 78
<br />Stat. 252, 42 U.S.C. 2000d to 2000-4 and
<br />Title 49, Code of Federal Regulations, De-
<br />partment of Transportation, subtitle A, Of-
<br />fice of the Secretary, Part 21, nondiscrimi-

<br />bidders that it will affirmatively insure that
<br />in any contract entered into pursuant to
