PERSONAL SERVICES CONTRACT <br />Architect and Engineer <br />BETWEEN: The City of Eugene, an Oregon <br />Municipal Corporation <br />AND: PBS Engineering + Environmental <br />Contract No.: 2010-00220 <br />This contract is being established per the terms and conditions of Personal <br />Services Master Contract #2009-00224 dated November 17, 2008, between the <br />City of Eugene (City) and PBS Engineering + Environmental (Contractor). <br />City and Contractor mutually agree to the attached scope of work as outlined <br />herein and the contract referenced above as follows: <br />Project Description: services are to complete hazmat environmental <br />documentation required by ODOT for the 11 th Avenue from Garfield <br />Street to Tyinn Street and 13th Avenue from Alder Street to Washington <br />Street ARRA pavement preservation projects. Consultant will provide <br />research and documentation prepared in a Level 1 Corridor Study to <br />obtain hazmat clearance with ODOT. <br />For Clarification, see Scope of Work attached as Exhibit B. Contract <br />total is not to exceed $8,000.00 Contract completion date is <br />December 31, 2010. <br />All other terms and conditions of master contract #2009-00224 apply to this <br />scope of work and remain in full force and effect. <br />
