PERSONAL SERVICES CONTRACT <br />Architect and Engineer <br />z <br />BETWEEN: The City of Eugene, an Oregon <br />Municipal Corporation <br />AND: Pavement Services, Inc. <br />Contract No.: 2010-00218 <br />This contract is being established per the terms and conditions of Personal <br />Services Master Contract #2009-00225 dated December 22, 2008, between the <br />City of Eugene and Pavement Services, Inc. <br />City and Contractor mutually agree to the attached scope of work as outlined <br />herein and the contract referenced above as follows: <br />Pavement design services for Patterson Street (GJN 4622). Start work <br />date of November 9, 2009, and contract termination date of January 30, <br />2010.
