9.1 Request. Requests for ground leases on the Airport, or for permission to carryon any <br />commercial aeronautical business to serve the public on the Airport, shall be made to the Airport <br />Manager by delivering all necessary permits and licenses to the Administration Office in triplicate. <br />The request shall be in sufficient detail to discern the qualifications of the applicant to perform the <br />desired service and shall include the following: <br />9.1.1 A written synopsis detailing the nature of the proposed activity including: <br />"` 9.1.1.1 The name, address, and telephone number of the applicant; <br />9.1.1.2 A detailed description of the proposed fixed-base operation that will <br />serve the public including the proposed date of commencement of services; <br />9.1.1.3 The professional qualifications of personnel who will manage and/or <br />operate the proposed fixed-base operation; <br />9.1.1.4 Descriptions and cost estimates of any proposed capital improvements <br />on the fixed-based operation site. <br />9.1.2 A current financial statement prepared or certified by an independent certified <br />public accountant. The Airport Manager shall consider financial statements in evaluating the <br />applicant's financial ability to provide responsible, safe, and adequate service to the public. <br />9.1.3 A written listing of the assets owned, leased, or being purchased which will be <br />used in the business on the Airport. Copies of any leases or purchase contracts must be <br />attached. <br />9.1.4 A written agreement that the applicant will execute such forms, releases, or <br />.discharges as may be .requested by the FAA and all aviation or aeronautic commissions, <br />administrators, or departments of all states ion which the applicant has engaged in aviation <br />'~ business, to release information in their files relating to the applicant or its operation. <br />9:1.5 The request shall be signed and submitted by the owner of the business if a sole <br />proprietorship, every partner if a partnership, and the President or CE0 if a corporation. <br />9.1.6 The applicant agrees to provide any additional information and material <br />necessary, or requested by the Airport Manager, to establish to the satisfaction of the Airport <br />Manager that the applicant can qualify and will comply with these minimum standards. <br />9.1.7 If directed by the applicant, the Airport Manager shall hold the financial <br />information included with the documents separate from the request and not available for public <br />inspection to the extent covered by the Oregon Public Information Act. <br />9.2 Fee. The request shall be accompanied by a fee set by the Airport Manager under <br />Sections 2.440 to 2.442 of the Eugene Code, l 971, effective December, l 993 as they exist now or may <br />hereafter be amended. <br />9.3 Review of Request. A request shall be allowed or denied within 45 days after receipt <br />of all of the required documents. The request maybe approved if <br />. 9.3.1 The requested operation is consistent with the Airport Master Plan, the <br />Eugene-Springfield Metropolitan Area General Plan, and relevant provisions of federal, state <br />and local laws and rules; and <br />9.3.2 The requested operation would enhance the health, welfare and safety of users <br />of the Airport. <br />Administrative Order - 29 <br />r:\adminord\rules\03airrl2ao.wpd(OS/16/03 ) <br />
