1. The fees set forth in the attached Exhibit A are the fees to be charged for the <br />described services as of the effective date of this Order. <br />2. The unamended fees established by Administrative Order No. 21-04-02-F remain <br />in full force and effect and are incorporated into the fee schedule attached as Exhibit A. <br />3. Administrative Order No. 21-04-02-F is repealed, and the provisions of <br />Administrative Order Nos. 44-92-05, 53-04-O1-F, and 57-02-08-F that are in conflict with the fee <br />schedule attached as Exhibit A are repealed, as of the effective date of this Order. <br />4.
