AMENDMENT NO. One TO <br />City of Eugene Contract No. 2009 - 00229 <br />This Amendment is made to the Personal /Professional Services Contract dated November 17, <br />2008, between the City of Eugene (City) and GeoScience, Inc. (Contractor). <br />City and Contractor mutually agree to amend and extend the contract referenced above as <br />follows: <br />Amendment One adding work on Upper Crest (see scope of work below) with a <br />cost not to exceed $1,960.00. Completion date to be extended to June 30, 2009. <br />SCOPE OF WORK: <br />Mark out borings for utilities locate: 1 hour <br />Install four hand auger borings: 5 hours <br />Draft Logs (with photos): 2 hours <br />Change Report Figures to include new info: 3 hours <br />Meeting to discuss recommendations: 3 hours <br />Total (not to exceed) cost = $ 1,960.00 <br />All other terms and conditions of this contract remain in full force and effect. <br />r/ CITY OF EUGENE <br />KurtlCorey , P.
