<br /> s <br /> PERSONAL SERVICES CONTRACT <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> AND: PBS Engineering + Environmental <br /> Contract No.: 2009-00234 <br /> This contract is being established per the terms and conditions of Personal ' <br /> Services Master Contract master contract #2009-00224 dated November 17, <br /> 2008, between the City of Eugene Public Works Engineering and PBS <br /> Engineering & Environmental. <br /> City and Contractor mutually agree to the attached scope of work as outlined <br /> herein and the contract referenced above as follows: <br /> Services are to complete hazmat environmental documentation required by <br /> ODOT for City of Eugene job number #4279, the West Bank Path Extension <br /> federal aid transportation enhancement project ODOT #24,796. PBS Engineering <br /> & Environmental will provide research and documentation in order to prepare a <br /> Level 1 Corridor Study and obtain hazmat clearance. Work to begin December 1, <br /> 2008 and to be completed no later than December 31, 2010. Contract amount <br /> not to exceed $7,500. <br /> For Clarification, see Scope of Work attached in Exhibit B <br /> All other terms and conditions of master contract #2009-00224 apply to this <br /> scope of work and remain in full force and effect. <br /> CERTIFICATION REGARDING USE OF CONTRACT FUNDS FOR LOBBYING <br /> (Applicable to all Federal-aid contracts and subcontracts which exceed $100,000 <br /> A. The prospective participant certifies, by signing and submitting this bid or <br /> proposal, to the best of his or her knowledge and belief, that: <br /> (1) No Federal appropriated funds have been paid or will be paid, by or on <br /> behalf of the undersigned, to any person for influencing or attempting to <br /> <br />
