PERSONAL SERVICES CONTRACT <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> AND: Pavement Services, Inc. <br /> Contract No.: 2009-00237 <br /> This contract is being established per the terms and conditions of Personal <br /> Services Master Contract #2009-00225 dated December 22, 2008, between <br /> the City of Eugene (City) and Pavement Services,-Inc. (Contractor). <br /> City and Contractor mutually agree to the attached scope of work as outlined <br /> herein and the contract referenced above as follows: <br /> Project description: perform pavement testing, analysis and <br /> design on various streets in Eugene listed in Exhibit B as part <br /> of the pavement preservation project testin_ g for FY2010. <br /> Contract completion date is December 31, 2011.
