City of Eugene <br /> Human Resource and Risk Services Policies <br /> 3-227.9 Access to Information <br /> Employees have the right to review their department employee file. Department <br /> employee files must be reviewed in the presence of a designated department staff <br /> member. <br /> Employees have the right to review their personnel file in HRRS. Personnel files <br /> must be reviewed in the presence of a designated staff member of Human <br /> Resource and Risk Services. Employees wishing to review their personnel file <br /> should contact Human Resource and Risk Services and make an appointment. <br /> Employees are required to show picture identification before reviewing their <br /> personnel file. <br /> An employee may authorize in writing access to his/her personnel file to his/her <br /> designated representative. Forms authorizing the release of records are available <br /> in Human Resource and Risk Services. Personnel files may not be removed from <br /> the Human Resource and Risk Services office. Upon request, and for a charge, <br /> employees maybe provided copies of documents in their personnel file. Certified <br /> copies of personnel file documents maybe provided only by Human Resource and <br /> Risk Services. Any requests for certified copies of personnel files must be in <br /> writing and should be referred to Human Resource. and Risk Services. <br /> 3-227.10 Employee Records Retention (Revised 09/16/02) <br /> The retention of documents in an employee's personnel file varies according to <br /> state law and collective bargaining contracts. <br /> Upon termination of employment any employee files maintained by the Finance <br /> Division and the employee's department/division should be sent to Human <br /> Resource and Risk Services when the termination PAF is sent. Human Resource <br /> and Risk Services will remove any superfluous or duplicate documents and the <br /> remaining significant documentation will be transferred to the City's official <br /> Personnel File. <br /> 3-228 Training, Education, and:Development <br /> The City encourages professional training, education, and development opportunities for <br /> employees with the objective of improving City services.
