This form is in the employee's personnel file. (A copy is on Page 32 of this ' <br /> section.) Do not give out any information unless this form has been signed. <br /> i <br /> ~ Frequently, supervisors are unsure how to respond to questions from other employers conducting reference checks on <br /> former City of Eugene employees. The following guidelines are an attempt to assist with this sensitive subject.
