After making decisions <br /> regarding your health insurance benefits, PERS, and deferred compensation, please complete and sian <br /> the attached Notice of Intent and Notice of Qualifyinq_Event forms and return them to your supervisor <br /> or office manager for attachment to your "Termination PAF." Please call me at x8868 (682-8868) if <br /> you have any questions about your retirement. <br /> Post-Retirement Health Insurance <br /> Your City-paid health insurance will cease at the end of the month in which you terminate employment. <br /> You have options of seeking health coverage from a private carrier, under ~~Portability" (if you are on <br /> the PacificSource Plan), through PERS, or retiree. continuation through the City. <br /> 1. Portabi/ity. If you are covered under PacificSource, you will receive ~~Portability" information <br /> directly from them. Do not confuse this information with Retiree Continuation materials. Portability <br /> coverage is provided by PacificSource, has different benefit levels and an age-based premium schedule. <br /> 2. PERS Option: PERS Health Insurance Program offers several medical plans and a dental plan for <br /> PERS retirees, including ODS Health Plans and Providence Health Plan. PERS also pays a subsidy <br /> toward the cost of health-care coverage for eligible retirees on Medicare (Parts A and B). For rates and <br /> health plan option information, contact PERS Health Insurance Program in Portland at 1-800-768-7377. <br /> 3. City of Eugene Retiree Continuation: Employees retiring under PERS may continue their health <br /> insurance coverage through the City of Eugene, on a self-pay basis, until eligible for Medicare. You <br /> have 60-days to elect this type of coverage. The basic provisions of post-retirement insurance <br /> coverage through the City of Eugene are as follows: <br /> .Eli ibili Retiree: Until eligible for Medicare (usually age 65) <br /> Spouse: Until eligible for Medicare (usually age 65) <br /> Dependents: Unmarried children, until age 18* <br /> *Eligible dependent children over age 18 have their own COBRA continuation rights at the time you retire. <br /> Contact me in HRRS for more information if you cover your dependent children. <br /> <br />
