Contact Risk Services for details. <br /> 7-321 Accident Reporting <br /> When an accident occurs, it must be reported immediately to the employee's <br /> supervisor. <br /> 7-321.1 Supervisor Responsibility (Revised 04 01 03) <br /> Supervisors are responsible for reporting to the Oregon OSHA field office <br /> (503)686-7562, all fatalities or catastrophes within eight (8) hours, and all <br /> accidents or injuries resulting in a hospital admission with medical treatment <br /> other than first aid, within twenty-four (24) hours after receiving notification. <br /> Catastrophe: An accident in which an employee is fatally injured, or three or <br /> more employees are admitted to a hospital or an equivalent medical facility. <br /> It is the responsibility of Risk Services to provide each Executive and Division <br /> Manager with a monthly summary of all reported injury accidents, and to <br /> maintain and annually post all reported injury accidents in accordance with OAR <br /> 437-01-275. <br /> A.
