This constitutes the award of Cooperating Technical Partners (CTP) <br /> program funding per the City of Eugene's application dated July 8, 2004 and <br /> Mapping Activity Statement dated July 22, 2004. <br /> 2. This funding may only be used for activities described in the Mapping <br /> Activity Statement. <br /> 3. Quarterly Financial Status Reports (FSR), FEMA Form 20-10, are due 30 <br /> . days after the end of each Federal quarter (January 30, Apri130, July 30 and <br /> October 30). A final FSR is required and is due 90 days after the <br /> .performance period expires. Reports should be sent to: Christine <br /> VanZweden, Grants Management Specialist, FEMA Region 10. <br /> 4. Quarterly Performance Reports, in narrative format that must include the <br /> Agreement number and report quarter are due 30 days after the end of each <br /> Federal quarter (January 30, Apri130, July 30 and October 30). A final <br /> Performance Report is due 90 days after the performance period expires. <br /> Reports should be sent to: Christine VanZweden, Grants Management <br /> Specialist, FEMA Region 10. <br /> 5. The funds awarded by this FEMA form 76-10A are available for <br /> obligation by the recipient only during the period August 1, 2004 through <br /> July 31, 2006. The recipient is not authorized to incur new obligations after <br /> the expiration date shown unless a new expiration date is established by <br /> FEMA through the issuance of a new FEMA form 76-10A. <br /> 6. The performance period for this Agreement is August 1, 2004 through <br /> July 31, 2006. <br /> 7. The attached Agreement Articles are a binding part of this Cooperative <br /> Agreement. <br /> <br />
