<br /> F' <br /> PERSONAL SERVICES CONTRACT _ <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> AND: Environmental Solutions, LLC. <br /> Contract No.: 2009-00233 <br /> This contract is being established per the terms and conditions of Personal <br /> Services Master Contract master contract #2009-00203 dated July 30, 2008, <br /> between the City of Eugene (City) and Environmental Solutions, LLC. <br /> City and Contractor mutually agree to the attached scope of work as outlined <br /> herein and the contract referenced above as follows: <br /> Complete biological services assessment and report, wetland and waterways <br /> assessment and permitting, and Joint Permit application, as required, for the <br /> West Bank Path Extension (River Avenue to Beaver Street) federal-aid <br /> Transportation Enhancement project (ODOT #24,796). Contract not to <br /> exceed $70,000.00. Contract completion date is December 31, 2010. <br /> For Clarification, see scopes of work attached as Exhibits B, <br /> C&D. <br /> All other terms and conditions of master contract #2009-00203 apply to this <br /> scope of work and remain in full force and effect. <br /> CERTIFICATION REGARDING USE OF CONTRACT FUNDS FOR LOBBYING <br /> (Applicable to all Federal-aid contracts and subcontracts which exceed $100,000 <br /> A. The prospective participant certifies, by signing and submitting this bid or <br /> proposal, to the best of his or her knowledge and belief, that: <br /> (t) No Federal appropriated funds have been paid or will be paid, by or on <br /> behalf of the undersigned, to any person for influencing or attempting to <br /> influence an officer or employee of any Federal agency, a Member of <br /> Congress, an officer or employee of Congress, or an employee of a <br /> Member of Congress in connection with the awarding of any Federal <br /> contract, the making of any Federal grant, the making of any Federal <br /> <br />
