This information is necessary for <br />preparation of annual reports required by the Federal Highway Administration and is essential in policy <br />and program development by FHVIIA. The questionnaire responses are shared with the League of <br />Oregon Cities and Association of Oregon Counties for compilation of reports satisfying Legislative <br />reporking requirements required by ORS 366.790 and 366.712. The county results are also needed to <br />help determine the distribution of the Special County Program funds. <br />Enclosed is the questionnaire for fiscal year 2008, with general and detailed instructions. Preaudited <br />numbers maybe used when completing these forms. <br />There are three main sections that need to be completed: <br />- Section 1 (Receipts for roads and streets) <br />- Section 2 (Disbursements for roads and streets) <br />- Section 3 Debt information for road and street bonds and notes. <br />As noted in the instructions, you only need fill out Section IV if you have jurisdiction for roads and <br />streets on the National Highway System. If this applies to your city or county, you will find the form <br />attached to your packet. <br />Please submit your survey by September 15, X008. Survey". <br />You will need an application called Acrobat Reader to open the file. <br />The program will open to the cover letter of the survey.
