The foll:owing provisions if applicable are.hereby included in and made a part <br />. of the attached contract' between the City of Eugene and the Contractor named <br />thereon as provided'-for.in the Eugene Code, 1971, the revised statutes of the <br />State of Oregon, and Federal laws, rules, regulations, and guidelines: <br />1. Fair Em 1o ent'Practice Provisions <br />Eugene Code, 1971, Section 4.625 <br />The following prov.isions are applicable to any contract with a Contrac- <br />' tor if the City purchases goods or services from the Contractor costing <br />$2,500 or more in any fiscal year: <br />~ <br />1.1 During the performance of this contract, the Contractor agrees as . <br />follows: - <br />1.1.1 The Gontractor will not discriminate against a <br />or applicant for employment because of raee, color, sex, age, national <br />origin, physi.cal or mental hand.icap, unless based upon a bonafide <br />occupational qualification. <br />1.1.2 Those Contractors employing 15 or more individ <br />develop and implement an affirmative action plan_to insure that appli- <br />cants are employed,, and that employees are treated during employment, <br />without regard~to their race, color, sex, age, or national origin. <br />Such plan shall include, but not be limited to the foll'owing: employ- <br />ment, upgrading, demotion, transfer, recruitment, recruitment adver- <br />tising, layoff or termination, rates of pay or other forms of compen- <br />sation, and selection for training, including apprenticeship. <br />1.1.3 The Contractor agrees to post in conspicuous p <br />able to employees and apPlicants for employment, notices to be provid- <br />ed by the Human Rights Gouncil setting forth the provisions of this <br />non-discrimination clause. <br />1.2 The Contractor will prior to commencement and during the term of <br />this contract: <br />1.2.1 Furnish to the Human Rights Council, upon requ <br />such form as may be provided by the Human Rights Council, a written <br />report of the affirmative action taken by the Contractor in implement- <br />ing the plan described in paragraph 1.1.2 within 30 days from the date <br />such a request is made; and <br />1•2•2 Permit the Human Rights Council to inspect and <br />records of employment, employment advertisements, application forms, <br />and other pertinent data and.,records of the Contractor, for the pur- <br />pose of investigation to determine compliance with paragraph 1.1. <br />1.3 If, upon inVestigation, the Human Ri.ghts Council finds reasonable <br />cause to believe the Contractor`or any subcontractors of the Contractor have <br />Personal Services Contract -.10. <br />
