.~, ~ City Recap U... - <br />Architect <br />Contractor <br />Project Manager <br />AMENDMENT N0.2 Copy/File <br />TO <br />City of Eugene Contract Na. 2006-03854 <br />This Amendment is made to the Personal Services Contract dated January 17, 2006, between the <br />City of Eugene and artists Betsy Wolfston and David Thompson. <br />The purpose of this Amendment is to provide additional services by the Artist Team, extend the <br />date of campletion, and provide compensation for the additional services. City and Contractor <br />mutually agree to amend the contract referenced above as follows: <br />2. Services to be Provided: Add the following item to include increa.ged services <br />" 2.7 Artist Team to provide work and services to coordinate with modifications to the base." <br />4. Time for Completion: <br />Amend Section 4.2 to change the completion date from November 30, 2006 to Apri130, 2007. <br />5. Consideration: <br />Amen Section S. to change the consideration from $18,000 to $18,718. <br />Add Section <br />"5.5 Phase 4: Payment for Additional Work by the Artist team. Both members of the Artist <br />team shall submit bills together to the City's Project Manager totaling $718.00.
