V PERSONAL SERVICES CONTRACT <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> <br /> ~ AND: Pacific Water Resources, Inc. <br /> 4905 S. W. Griffith Drive, Suite 200 <br /> Beaverton, Oregon 97005 <br /> Contract No.: 2006-00234 <br /> This contract is being established per the terms and conditions of Personal <br /> Services Master Contract master contract #2006-00205 dated August 2, 2005, <br /> between the City of Eugene (City) and Pacific Water Resources, Inc. (Contractor). <br /> City and Contractor mutually agree to the attached scope of work as outlined <br /> herein and the contract referenced above as follows: <br /> Project Description: Hydrologic analyses were previously submitted as part <br /> 2a. Phase 2b will complete the final phase of the Lower Amazon Study <br /> and will include the final packaging and submittal for the hydraulic informa- <br /> tion, including mapping and coordination with FEMA for that submittal. <br /> Once the information is submitted, a review will likely result in coordina- <br /> tion's and communications with FEMA to resolve any issues noted in the <br /> review. Phase 2b will result in acceptance of the study and a mapping <br /> revision. Contract not to exceed $28,910.00 with a completion date of <br /> April 1, 2007. <br /> For Clarification, see Scope of Work and Fee Schedule attached. <br /> All other terms and conditions of master contract #2006-00205 apply to this scope <br /> of work and remain in full force and effect. <br /> <br />
