Five TO <br /> City of Eugene Contract No. 2005-00208-B <br /> This Amendment is made to the Personal Services Contract dated November 16, 2004, <br /> between the City of Eugene (City) and CH2M HILL (Contractor). <br /> The City and Contractor mutually agree to extend the contract referenced above as follows: <br /> Extend completion date of final design of the Polk Street Outfall Quality <br /> Improvements Project #3984 (2005-00208-B) until December 31, 2007 and add: <br /> Status Meetings: CH2M HILL will provide one staff person to attend up to twelve <br /> (12)1-hour project status meetings. Meetings will occur by conference call. <br /> CH2M HILL will prepare brief meeting notes for each meeting held, identifying key <br /> discussion, decisions, and action item assignments. Attendance at these meetings <br /> and preparation of notes is expected to require 161abor hours. <br /> Authorized Fee <br /> Total authorized fee will be increased by $1,650.00 for these additional services based on the <br /> original pool contract pricing. <br /> All other terms and conditions of this contract remain in full force and effect. <br /> CITY OF EUGENE CONTRACTOR <br /> Kurt Corey, P.E.
