This request covers additional meetings and services not covered in the previous request. This request will cover all of the MIG's tasks through the end of December according to our current schedule. 1. December MAC meeting: The previous extension included 4 additional meetings, which will cover the cost of the June, September, October, and November 2004 MAC meetings. The cost for this meeting is 8 Principal hrs C $135, for a total of $1,080. 2. Coordination meetings: The previous extension included 4 additional meetings, which will cover the cost of the June, August, September, and October coordination meetings. Coordination meetings for November and December are needed, at a cost of $400 per meeting. This is a total of $800. 3. Project management: Project management funds were expended in September. Funding for project management activities is needed for October, November, and December at the contract rate of $316/month. This totals $948. 4. Previously approved financial analysis extension: The City previously approved $2,648 for recalculating the financial analysis using the 04 budget figures. As I mentioned, this was omitted from the June 04 budget request. 5. • Previous shortage: The previous extension for $20,000 included a shortage of $115. This request includes the $115.
