PERSONAL SERVICES CONTRACT <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation (City) <br /> AND: R-J Consulting Services, LLC (Contractor) <br /> CONTRACT NO.: 2005 - 05409 <br /> RECITALS <br /> A. Contractor is engaged in the business of Vegetation Management. <br /> B. City desires to engage Contractor to provide the services described in this agreement and <br /> Contractor is willing to provide such services on the terms and conditions set forth herein. <br /> C. The following are attached and incorporated as a part of this contract: <br /> Exhibit A -City of Eugene Standard Contract Provisions <br /> Exhibit B -Scope of Services <br /> Exhibit C -Schedule of Rates <br /> <br /> ~1r AGREEMENT <br /> 1. Services to be Provided. Contractor agrees to provide to City the following services (°the <br /> Work"): <br /> The statement of Work is contained in the attached Exhibit B <br /> 2. Time for Completion. Contractor shall commence the Work on July 6, 2004, and shall <br /> complete the Work prior to July 6, 2005. This contract may be renewed for four (4) <br /> additional one-year periods by mutual agreement of City and Contractor. <br /> 3. Consideration. The consideration which City shall pay to Contractor, for both the work <br /> performed by Contractor and the expenses incurred by Contractor in performing the Work, <br /> shall not exceed $25,000.00 based on the schedule of rates described in the attached <br /> Exhibit C. City shall make progress payments based on invoices from Contractor, together <br /> with a progress report containing a summary of the Work completed for which payment is <br /> requested. City's payment shall be made within 30 days after Contractor's statement. <br /> Contractor shall not be entitled to reimbursement for travel or other expenses unless <br /> specified on the attached schedule of rates. If Contractor is entitled to reimbursement for <br /> any such expenses, they shall be properly documented and submitted in accordance with <br /> City requirements. <br /> 4. Termination.
