~ <br />PERSONAL SERVICES CONTRACT <br />Architect and Engineer <br />BETWEEN: The City of Eugene, an Oregon <br />Municipal Corporation <br />AND: Pavement Services, Inc. <br />Contract No.: 2008-00205 <br />This contract is being established per the terms and conditions of Personal <br />Services Master Contract master contract #2003-00216 dated November 21, <br />2002, between the City of Eugene (City) and Pavement Senrices, Inc. <br />(Contractor). <br />City and Contractor mutually agree to the attached scope of work as outlined <br />herein and the contract referenced above as follows: <br />~ <br />Project Description: Provide pavement testing and design <br />recommendations for Minda Drive from Gilham to Norkenzie, <br />1 St Avenue from Seneca to Bertelsen, 5th Avenue from McKinley <br />to Bailey Hill, and 7"' Place from 7th Avenue to Bailey Hill. Contract <br />not to exceed $60,000 with a completion date of July 1, 2009. <br />For Clarification, see Scope of Work attached as Exhibit B. <br />All other terms and conditions ot master contract #2003-00216 apply to this <br />scope of work and remain in full force and effect. <br />~ CITY OF EUGENE <br />~~ ~ <br />K Corey, P.
