PERSONAL SERVICES CONTRACT <br />Architect and Engineer <br />BETWEEN: The City of Eugene, an Oregon <br />Municipal Corporation <br />AND: GeoDesign Inc <br />Contract No.: 2008-00210 <br />This contract is being established per the terms and conditions of Personal <br />Services Master Contract master contract #2007-00204 dated July 25, 2006, <br />befinreen the City of Eugene (City) and GeoDesign Inc. (Contractor). <br />City and Contractor mutually agree to the attached scope of work as outlined <br />herein and the contract referenced above as follows: <br />Project Description: Provide pavement testing and design <br />recommendations for City View Street from W. 18th Avenue <br />~- to W. 28th Avenue, High Street from E. 3`d Avenue to E. 11 "' <br />Avenue, Hilyard Street from E. Broadway to E. 13th Avenue, <br />Lincoln Street from W. 23~d Avenue to W. 30~" Avenue, E. 33~d <br />Avenue from Hilyard Street to Willamette Street, E. 13~h Avenue <br />from Kincaid Street to Alder Street, and Alder Street from E. 13~' <br />Avenue to E. 18th Avenue. In addition, GeoDesign will prepare a <br />cold-in-place recycling rehabilitation design alternative from <br />Roosevelt Blvd from Chambers Street to Beltline. Project to be <br />completed by September 30, 2009 with a maximum total contract <br />amount not to exceed $60,000.00. <br />For Clarification, see Scope of Work attached as Exhibit B. <br />All other terms and conditions of master contract #2007-00204 apply to this <br />scope of work and remain in full force and effect. <br />
