~it~fi ~. =~t~~~ E~~~s~~~/Rehabilitation
^ For temporary disturbance of soils and/or vegetation in waterways, wetlands or riparian areas, please discuss how you
will restore the site after construction including any monitoring, if necessary *
Site restoration will include seeding with native grasses and forbs, plantings of bare root native vegetation such as
willow, dogwood, ash, cottenwood, maple....(see plant list) Plugs of aquatic or emergent vegetation will be
planted in areas of shallow water to enhance habitat for odantes and other aquatic life. Use of large wood will be
placed in locations to improve habitat for both Western Pond Turtle and other wildlife and fish species.
Development of open sandy south facing beach or bank habitat will be managed to encourage western pond turtle
nesting habitat. Areas above Ordinary High Water will be surfaced with a cover of organic PH neutral mulch for
erosion control and nutriant additives. In areas needing more protection from soil erosion, mulch berms will be
adapted to the land form to create an erosion barrier to waterways. Monitoring of vegetation succession will
occur for two years as art of the ical warran of the contract.
Miti ation
Describe the reasonably expected adverse effects of the development of this project and how the effects will be mitigatecl. *
^ For permanent impact to wetlands, complete and attach a Compensatory Wetland Mitigation (CWM) Plan. (See DAR
141-OSS-0.121 to C)~1R 141-(185-0176 for plan requirements) *
^ For permanent impact to watenvays or riparian areas, complete and attach a Compensatory Mitigation (CM) plan (See
OAR 141-08~-0115 for plan requirements) *
^ For permanent impact to estuarine wetlands, you must submit an Estuarine Resource Replacement Plan. (See 0.4.R .14J-
f185-t)240 to C1~~.R 1=F.1-OF~.5-t12~7forplan requiYements)*
Adverse impacts to this site are anticipated to be temporary . While the land form will permanently be altered, the alteration
will provide positive ecological benefits to the ponds and adjacent sites and be considered enhancements. It is anticipated that
mitigation should not be necessary since the project goals and direction are specifically designed to provide large scale
enhancements to this site for all natural resource elements.
itl Action LOCation In oYmati0lt (Fill out only when mitigation is proposed or required)
Proposed mitigation: ^Onsite Mitigation Type of mitigation:
(Check all that apply) ^Ofj°site Mitigation ^ Wetland tLlitzgation
OMitigation Bank ~Mitigation for impacts to other waters
^Pa ment to Provide ~Miti ation or im acts to navi ation, zshin , or recreation
Street, Road or other descriptive location Le al Descri tion attach tczx 1~t mcz *
Quarter/Quarter Section Township Range
In or Near (Ciry or Town) Coun Tax Ma # Tax Lot #3
WetlandlWaterwayName (pickone) RiverMile (ifknown) 1';c~titarde (in DD.DDDD formaf) l;vn~itrrde (in DD.DDDD format)

Name of watenvay/watershed/fIZIC Name of mitzgation bank ('f applicable)
3 Attach a copy of all tax maps with the project area higYGghted.
Page 6
