EUGENE SCHOOL DISTRICT 4J <br /> Community Use of (Non- Reserve, Non - Surplus) School Facilities Guidelines <br /> August 1, 1996 July 1. 2001 <br /> Administrative Procedures: The principal or designee will provide the prospective user with a copy of the <br /> "Community Use of School Facilities Guidelines" and a current application (form 7 -B). The Guidelines by <br /> reference become part of the lease agreement (form 7 -A) when a lease is signed by the user and the School <br /> District. All questions pertaining to use of school facilities not covered in this document, should be referred <br /> to the Facilities Management Rental Office, phone 687 -3550. <br /> A 7 -B form must be completed by the requesting group, department, or individual, and the building <br /> principal/designee for all activities in all classifications except school sponsored activities for students and <br /> parents (Class I. A & B). All school related groups, community school groups, 4J cosponsored activities, <br /> parks & recreation department activities, and community youth organizations, must complete a 7 -B form <br /> (including child care or day care groups using school facilities.) <br /> All groups and /or individuals using 4J facilities for non -fee. including those that are school sponsored. or <br /> approved youth groups using facilities for a nominal (maintenance) fee. must adhere to all District policies. <br /> In addition. the group's local. regional (if any). and national (if any) policies must comply with all District 4J <br /> policies. It is the responsibility of the principal or administrator sponsoring or renting to such a <br /> group /individual to ensure that these requirements are met. <br /> The District shall not sponsor or continue the sponsorship of any individual or group whose local. regional or <br /> national policy or practice does not comply with the District's non - discrimination and harassment policy. <br /> Further, all individuals or groups sponsored by the District shall agree to comply with the District's non- <br /> discrimination and harassment policy. <br /> It is the principal's responsibility to ascertain that the proposed activity: <br /> 1. Will not violate District or OSAA policies <br /> 2. Will not be harmful to the school facility <br /> 3. Will not interrupt the school's programs <br /> 4. Will appropriately fit into the building use calendar <br /> 5. Will have adequate and appropriate supervision <br /> 6. Will not cause or allow a breach in security <br /> It is also the principal's responsibility to see that: <br /> 1. The applicant completes in full and signs Part 1 of the 7 -B form. <br /> 2. The principal /designee completes in full and signs Part II of the 7 -B form. <br /> 3. Each 7 -B form is forwarded to Facilities Mgt. for information and execution of a lease agree- <br /> ment (7 -A form) if one is required. <br /> 4. 7 -B forms for Class I activities are sent to Facilities Mgt. prior to the event. <br /> 5. Class II and III applicants are not allowed to use facilities until the school receives a fully <br /> executed copy of the form 7 -A lease agreement. (Applicant may be required to hand carry the <br /> school- approved, fully completed 7 -B form to Facilities Mgt. (715 West 4th) to obtain a lease, <br /> when necessary in order to meet this requirement.) <br /> 6. An appropriate supervisor is selected and trained in facility use rules and regulations, proper <br /> facility security, safety and supervision procedures. <br /> 1 <br />
